SPINAL OUTREACH SERVICE

The Spinal Outreach Service was established in July 2003. It is a NSW Department of Health Initiative to address the ongoing and complex needs of the person with Spinal Cord Injury, following discharge from hospital. The service offers it’s resources to clients and clinicians living in Metropolitan and Rural areas of New South Wales.

Referral Criteria

For Sydney Metropolitan clients admitted to hospital either acutely or following a readmission to hospital, referral is from the spinal units and is usually automatic.

Clients living in rural areas can contact us for advice via their clinician (GP, Community Nurse, Occupational Therapist)

Client Related

The multidisciplinary team will work together with the client, carer, clinicians and family to assist in the period of transition from hospital to home. Our aims are to assist with health issues as they arise, provide education and to facilitate community participation in terms of work, sport or leisure. The clinicians will meet with the clients.
